If you’re somewhere in that second group or just starting to notice changes you may be wondering whether hormone replacement therapy (HRT) is worth considering.

Here’s an honest look at what HRT is, who it tends to help, and how to figure out if it’s the right path for you.

Understanding the Menopause Transition

Menopause is the point at which a woman’s body is no longer capable of reproducing, marked by her final menstrual period. Perimenopause is the transitional stage leading up to that point, often marked by periods becoming longer, shorter, or skipped altogether. Postmenopause refers to the years after menopause has occurred.

Throughout this transition, estrogen and progesterone: the hormones that regulate the menstrual cycle fluctuate and gradually decline. This hormonal shift is responsible for many of the symptoms associated with menopause, including hot flashes, bone density changes, and vaginal dryness.

What Is Hormone Replacement Therapy?

HRT is a treatment that replaces the estrogen and in many cases, progesterone or progestin that the body no longer produces in sufficient amounts. For women who still have a uterus, progesterone or progestin is typically added to estrogen therapy to protect the uterine lining. For women who have had a hysterectomy, estrogen alone is often sufficient.

What Symptoms Can HRT Help With?

Hormone therapy has been shown to provide relief for a range of menopausal symptoms, including:

  • Hot flashes and night sweats
  • Vaginal dryness and discomfort during intercourse
  • Sleep disruption related to hormonal fluctuation

  • Do I have a personal or family history of breast cancer, blood clots, or heart disease?
  • Have I had a hysterectomy, and if not, do I need combined estrogen-progestin therapy?
  • Am I more interested in systemic treatment (for hot flashes and bone health) or localized treatment (for vaginal symptoms specifically)?
  • What delivery method: pills, patches, or other options fits best with my lifestyle?

If HRT Isn’t the Right Fit, You Still Have Options

Hormone therapy isn’t the only path to managing menopausal symptoms, and it’s not the right choice for every woman; particularly those with certain health risk factors. Non-hormonal medications, lifestyle adjustments, and targeted treatments for specific symptoms (like vaginal dryness or urogynecologic concerns) are all part of the broader toolkit your provider can discuss with you.

It’s also worth noting that any bleeding after menopause is not expected and should always be evaluated. If you experience postmenopausal bleeding, reach out to your provider promptly, regardless of whether you’re using hormone therapy.
